NIAAA administers the Chronic Disease Self Management Program (CDSMP) in Boone, Carroll, Dekalb, Jo Davies, Lee, Ogle, Stephenson, Whiteside and Winnebago counties. The CDSMP was designed by Stanford University and is a course of six sessions designed to help older persons manage their chronic disease.
